ABSTRACT Objective: to know the experience of the nursing team when acting in situations of parental bereavement in Neonatal Intensive Care. Method: qualitative study developed through semi-structured interviews from March to April 2021 with nursing professionals from a Neonatal Intensive Care Unit in Rio Grande do Sul, Brazil. The results were submitted to thematic content analysis. Results: The act of experiencing parental bereavement by the nursing staff involves sadness, helplessness, and suffering. Strategies faced at this moment are based on empathy and humanization. For the professionals, the knowledge to face the situation comes from their own experiences of loss in their personal and professional lives, from their strength and religiosity. Conclusion: it is expected that this study contributes to the practice of health professionals in assisting parents who experience the process of parental bereavement in neonatal intensive care.

RESUMEN Objetivo: conocer la experiencia del equipo de enfermería al actuar en situaciones de aflicción parental en Cuidado Intensivo Neonatal. Método: estudio cualitativo desarrollado a través de entrevistas semiestructuradas en el período de marzo a abril de 2021 con profesionales de enfermería de una Unidad de Cuidado Intensivo Neonatal en Rio Grande do Sul, Brasil. Los resultados se sometieron a un análisis temático de contenido. Resultados: el acto de vivir lo luto parental por parte del equipo de enfermería atraviesa la tristeza, la impotencia y el sufrimiento. Las estrategias a las que nos enfrentamos en este momento se basan en la empatía y la humanización. Para los profesionales, el conocimiento para afrontar la situación viene dado por las propias experiencias de pérdida en la vida personal y profesional, de la fuerza y la religiosidad. Conclusión: se espera que este estudio contribuya a la práctica de los profesionales de la salud en la asistencia a los padres que experimentan el proceso de luto parental en el contexto de los cuidados intensivos neonatales.

RESUMO: Objetivo: conhecer a vivência da equipe de enfermagem ao atuar em situação de luto parental na Terapia Intensiva Neonatal. Método: estudo qualitativo desenvolvido por meio de entrevistas semiestruturadas no período de março a abril de 2021 com profissionais de enfermagem de uma Unidade de Terapia Intensiva Neonatal no Rio Grande do Sul, Brasil. Os resultados foram submetidos à análise temática de conteúdo. Resultados: o ato de vivenciar o luto parental pela equipe de enfermagem perpassa pela tristeza, impotência e sofrimento. Estratégias enfrentadas nesse momento são pautadas na empatia e humanização. Para os profissionais, o conhecimento para encarar a situação advém das próprias experiências de perda na vida pessoal e profissional, da força e religiosidade. Conclusão: espera-se que este estudo contribua para a prática dos profissionais da saúde na assistência aos pais que vivenciam o processo de luto parental no contexto da terapia intensiva neonatal.

RESUMO Objetivo: traduzir, adaptar culturalmente e validar a escala Full Outline of UnResponsiveness para o português do Brasil. Método: estudo metodológico realizado no Hospital de Clínicas da Universidade Federal do Triângulo Mineiro, Uberaba, Brasil, por meio das etapas: tradução, síntese, avaliação pelo comitê de especialistas, retrotradução, consenso, avaliação semântica e pré-teste. Alcançou-se uma amostra de 188 pacientes adultos. A coleta de dados ocorreu entre agosto e dezembro de 2020. Analisou-se a validade de critério concorrente comparando a escala Full Outline of UnResponsiveness com a Escala de Coma de Glasgow por meio dos coeficientes de correlação de Spearman e Pearson, e a validade preditiva com a Regressão de Cox, Sensibilidade e Especificidade e Área Sob a Curva Receiver Operating Characteristic. Adotaram-se, também, o alfa de Cronbach e os coeficientes Kappa ponderado e de Correlação Intraclasse para a confiabilidade interobservador. Resultados: o teste de Spearman para os itens resposta motora e ocular, respectivamente, resultou-se em 0,81 e 0,96, e o de Pearson para o escore total em 0,97. Obteve-se um risco relativo de 0,80, especificidade de 95,5%, sensibilidade de 51,6% e acurácia de 0,80 (IC95%: 0,688-0,905, p<0,001). O alfa de Cronbach foi de 0,94, o Kappa ponderado variou entre 0,89 e 1,0 e o ICC resultou em 0,99. Conclusão: a escala Full Outline of UnResponsiveness - versão brasileira, manteve quatro domínios e os 20 itens da escala original, tornando-se apropriada para utilização no Brasil e contribuindo para a avaliação do nível de consciência e prognóstico de pacientes adultos em condição grave.

ABSTRACT Objective: to translate, culturally adapt and validate the Full Outline of UnResponsiveness scale into Brazilian Portuguese. Method: a methodological study carried out at the Clinical Hospital of Universidade Federal do Triângulo Mineiro, Uberaba, Brazil, through the following stages: translation, synthesis, evaluation by the experts' committee, back-translation, consensus, semantic evaluation and pre-test. A sample of 188 adult patients was reached. Data collection took place between August and December 2020. Concurrent criterion validity was analyzed by comparing the Full Outline of UnResponsiveness scale with the Glasgow Coma Scale by means of Spearman's and Pearson's correlation coefficients; and predictive validity analysis was performed with Cox Regression, Sensitivity and Specificity and Area Under the Receiver Operating Characteristic Curve. The Cronbach's alpha, weighted Kappa and Intraclass Correlation coefficients were also adopted for interobserver reliability. Results: Spearman’s test for the motor and eye response items, respectively, resulted in 0.81 and 0.96, and Pearson's test for the total score was 0.97. A relative risk of 0.80, 95.5% specificity, 51.6% sensitivity and accuracy of 0.80 (95% CI: 0.688-0,905, p<0.001) were obtained. Cronbach's alpha was 0.94, weighted Kappa varied from 0.89 to 1.0, and ICC resulted in 0.99. Conclusion: the Full Outline of UnResponsiveness scale (Brazilian version), maintained four domains and the 20 items from the original scale, making it appropriate for use in Brazil and contributing to the assessment of the level of consciousness and prognosis of adult patients in severe conditions.

Se llegó a una muestra de 188 pacientes adultos. La recolección de datos tuvo lugar entre agosto y diciembre de 2020. Se analizó la validez de criterio concurrente comparando la escala Full Outline of UnResponsiveness con la Escala de Coma de Glasgow por medio de los coeficientes de correlación de Spearman y Pearson, y el análisis de la validez predictiva se efectuó con la Regresión de Cox, Sensibilidad y Especificidad y Área por debajo de la Curva Receiver Operating Characteristic. También se adoptaron el alfa de Cronbach y los coeficientes Kappa ponderado y de Correlación Intraclase para determinar la confiabilidad interobservador. Resultados: en la prueba de Spearman para los ítems de respuesta motora y respuesta ocular, respectivamente, se obtuvieron valores de 0,81 y 0,96, y el coeficiente de Pearson para la puntuación total fue de 0,97. Se obtuvo un riesgo relativo de 0,80, especificidad del 95,5%, sensibilidad del 51,6% y precisión de 0,80 (IC 95%: 0,688-0,905, p<0,001). El alfa de Cronbach fue de 0,94, el índice Kappa ponderado varió entre 0,89 y 1,0 y el resultado del ICC fue 0,99. Conclusión: la escala Full Outline of UnResponsiveness (Versión brasileña), mantuvo cuatro dominios y los 20 ítems de la escala original, lo que la hace apropiada para ser utilizada en Brasil y contribuye a la evaluación del nivel de consciencia y del pronóstico de pacientes adultos en condiciones de gravedad.

Abstract Peach palm flours prepared from the landraces microcarpa (MIF), mesocarpa (MEF), and macrocarpa (MAF) were assessed for their functional and morphological characteristics. The flours had moisture contents in accordance with the current legislation, considerable lipid (≥3.95%) and carbohydrate (≥66.68%) contents, and potential as an energy source (≥300 kcal/100 g). MEF and MAF can be considered rich in dietary fibers since their fiber contents were above 6 g/100 g, which contributes to the nutritional enrichment of products prepared with those flours. The assessment of the technological properties, WAC (≥255%) and OAC (≥65%) showed the peach palm flours can be used as raw material in different products such as breads, cakes, meat products, and breaded foods, thus adding energy, nutritional, and economic value. It is worth pointing out that peach palm is gluten free, hence it is an option for the development of new products targeted at consumers with celiac disease.

Abstract INTRODUCTION Salmonella enterica serovar Panama belongs to the D1 serogroup and is frequently associated with nontyphoidal salmonellosis in humans. This study aimed to characterize isolates collected from Northeast Brazil by phenotypic and molecular methods. METHODS Forty four S. Panama strains were examined for antimicrobial susceptibility, virulence genes, and pulsed field gel electrophoresis (PFGE) types. RESULTS All strains were susceptible to antibiotics (except for streptomycin), presented classical virulence factors, and could be clustered into four groups and 18 pulsotypes. CONCLUSIONS This work calls for continuous surveillance for the emergence of antibiotic resistance and new clones in a geographical area.

Abstract Objective To analyze outcomes and associated factors in ischemic stroke patients submitted to thrombolytic therapy. Methods This was a retrospective cohort study of ischemic stroke patients submitted to thrombolytic therapy. Comorbidities, neurologic deficits and time of thrombolysis were described. The chi-squared test was used to assess association among comorbidities, time of thrombolysis, and occurrence of hemorrhagic transformation. Results There was a high frequency of comorbidities. Mean neurological deficit score was 15 points. Mean time window was 98 minutes and needle-to-door time, 89.9 minutes. Hemorrhagic transformation was observed in 20 patients. Bivariate analysis revealed that hemorrhagic transformation was associated with greater neurologic deficit score, atrial fibrillation and heart disease. Neurologic deficit fell from 51% to 12.5% between admission and discharge. Conclusion Thrombolytic therapy presented positive outcomes, regardless of long thrombolysis time and high neurologic deficit scores.

Resumo Objetivo Analisar desfechos e fatores associados em pacientes com acidente vascular cerebral isquêmico após terapia trombolítica. Métodos Estudo do tipo coorte retrospectivo de pacientes com acidente vascular cerebral isquêmico submetidos à terapia trombolítica. Foram descritas as comorbidades; os défices neurológicos e os tempos de atendimento. Utilizou-se o teste qui quadrado para associação entre comorbidades, tempos de atendimento e ocorrência de transformação hemorrágica. Resultados Houve elevada frequência de comorbidades. Défices neurológicos pontuaram média de 15 pontos. A janela de tempo obteve média de 98 minutos e o tempo porta-agulha, 89,8 minutos. Observou-se transformação hemorrágica em 20 pacientes. Na análise bivariada, a ocorrência de transformação hemorrágica esteve associada com maior défice neurológico, fibrilação atrial e cardiopatia. Houve redução dos défices neurológicos de 51% para 12,5 entre a admissão e alta. Conclusão A terapia trombolítica apresentou resultados positivos, apesar de tempos de atendimento elevados e pacientes com défices neurológicos com elevada pontuação.

The aim of the present study was to assess the prevalence of Haemophilus influenzaetype b (Hib) nasopharyngeal (NP) colonisation among healthy children where Hib vaccination using a 3p+0 dosing schedule has been routinely administered for 10 years with sustained coverage (> 90%). NP swabs were collected from 2,558 children who had received the Hib vaccine, of whom 1,379 were 12-< 24 months (m) old and 1,179 were 48-< 60 m old. Hi strains were identified by molecular methods. Hi carriage prevalence was 45.1% (1,153/2,558) and the prevalence in the 12-< 24 m and 48-< 60 m age groups were 37.5% (517/1,379) and 53.9% (636/1,179), respectively. Hib was identified in 0.6% (16/2,558) of all children in the study, being 0.8% (11/1,379) and 0.4% (5/1,179) among the 12-< 24 m and 48-< 60 m age groups, respectively. The nonencapsulate Hi colonisation was 43% (n = 1,099) and was significantly more frequent at 48-< 60 m of age (51.6%, n = 608) compared with that at 12-< 24 m of age (35.6%, n = 491). The overall resistance rates to ampicillin and chloramphenicol were 16.5% and 3.7%, respectively; the co-resistance was detected in 2.6%. Our findings showed that the Hib carrier rate in healthy children under five years was very low after 10 years of the introduction of the Hib vaccine.

Este trabalho teve por objetivo geral desenvolver um Sistema de Informações Geográficas (SIG), que permita o armazenamento de informações e a localização geográfica de apiários em oito municípios da região oeste do Paraná; estudar a flora local, a ocupação do solo; e a produção de mel na safra de 2010, em dois deles: Marechal Cândido Rondon e Santa Helena. Para isso foi utilizado o software SPRING, delimitando um raio de ação das abelhas de três quilômetros ao redor dos apiários. Foram entrevistados e cadastrados 126 apicultores e 383 apiários. Pelas imagens foram selecionadas áreas com maior e menor sobreposição de colmeias em Marechal Cândido Rondon (144 e 44 colmeias, respectivamente) e Santa Helena (165 e 40 colmeias, respectivamente), em três quilômetros de raio, selecionando 15 colônias em cada área, para o estudo dos parâmetros citados. Na análise multivariada de agrupamento foram formados cinco grupos, pela semelhança de manejo, indicando maior produção média nas colmeias da área mais povoada de Santa Helena e menor na área mais povoada de Marechal Cândido Rondon. O agrupamento das colmeias, as diferenças nas produções de mel e o levantamento florístico indicaram que estas diferenças podem estar associadas ao manejo, diferenças florísticas e climáticas registradas no período de produção nas áreas estudadas.

This study aimed to develop a Geographic Information System (GIS), for storage of information and geographic location of apiaries in eight counties in western Paraná; study the local flora; the land used; and the honey productivity in the harvest of 2010 in two of these areas: Marechal Cândido Rondon and Santa Helena. In order to do so we used the software SPRING, delimiting a radius of action of bees of three kilometers around the apiaries. We interviewed and registered 126 beekeepers with 383 apiaries. By using the images we selected areas with greater and lower overlap of hives in Marechal Cândido Rondon (144 and 44 hives, respectively) and Santa Helena (165 and 40 hives, respectively), in a three kilometers radius, selecting 15 colonies in each area, for the study of the parameters cited. In the multivariate analysis of the grouping, five groups were formed, by their similarity of management, indicating the higher average production in the hives of the most populated area of Santa Helena and lower average production in the most populated of Marechal Cândido Rondon. The grouping of hives, the differences in the production of honey and floristic survey indicated that these differences could be associated with management, floristic and climatic differences recorded in the period of production, in the areas studied.

O objetivo deste estudo de caso foi relatar e analisar o caso de um paciente afásico adulto em tratamento fonoaudiológico na clínica escola do Curso de Fonoaudiologia de uma instituição federal de ensino. O sujeito estudado é um paciente do sexo masculino, com 68 anos de idade, em tratamento na clínica escola desde maio de 2009, sendo que seu processo terapêutico foi analisado até dezembro de 2010. Quanto à história clínica, o paciente sofreu acidente vascular encefálico isquêmico no hemisfério esquerdo, em janeiro de 2009, quando repentinamente apresentou um quadro de mutismo. Após serem realizadas avaliações fonoaudiológicas, concluiu-se que o mesmo apresentava como hipótese diagnóstica "Afasia de expressão, oral e escrita, decorrente de acidente vascular encefálico isquêmico." Diante disto foi elaborado o planejamento terapêutico. Desde o início do processo de terapia foi possível observar evoluções linguísticas no paciente. O fato de o sujeito ter procurado atendimento fonoaudiológico 15 dias após o acometimento pelo acidente vascular encefálico, provavelmente foi um fator fundamental para sua boa recuperação, pois há um grau de recuperação espontânea em pacientes afásicos, porém a melhora é mínima sem o tratamento adequado. Em aproximadamente um mês de terapia fonoaudiológica, o paciente expandiu consideravelmente seu vocabulário. Apesar de a área lesada ser extensa e fundamental para a linguagem, observou-se avanços bastante positivos com a terapia. Deste modo, acredita-se que a intervenção fonoaudiológica precoce contribuiu, em grande parte, para a reorganização da atividade linguística, visto que o paciente comunica-se satisfatoriamente considerando a grande extensão de sua lesão.

The purpose of this article was doing a report case of an aphasic client subjected to speech therapy in the clinic of a Speech Pathology and Audiology course of a federal university. The studied subject is a male patient, aged 68, treated in the clinic since May 2009. His therapeutic process was analyzed up to December 2010. The client suffered an ischemic stroke in the left cerebral hemisphere, on January 2009, when suddenly presented mutism. After the evaluations, the therapist concluded that the client presented "aphasia, compromising the oral and the writing expression, caused by ischemic stroke". So, the treatment plan was created. Since the start of therapy, significant linguistic changes could be observed in the client. A primary factor for the satisfactory recover was the quick search for the speech treatment (15 days after the stroke), because there is a degree of spontaneous recovery in aphasic clients. But, the good results are minimal without the adequate therapy. Only one month after the beginning of the therapy, the client improved substantially his vocabulary. Despite the injury area is extensive and fundamental to the language, it was observed very positive results with the therapy. So, we believe that the early speech therapy contributed a lot to the linguistic recover, since the client communicate satisfactorily, if we consider the great extension of his injury.

In this study we evaluated the antibacterial activity of the crude hydroalcoholic extracts, fractions, and compounds of two plant species, namely Rosmarinus officinalis and Petroselinum crispum, against the bacteria that cause urinary tract infection. The microdilution method was used for determination of the minimum inhibitory concentration (MIC) and minimum bactericidal concentration (MBC). The crude hydroalcoholic extract of R. officinalis displayed in vitro activity against Gram-positive bacteria, with satisfactory MBC for the clinical isolate S. saprophyticus. The fractions and the pure compound rosmarinic acid did not furnish promising results for Gram-negative bacteria, whereas fractions 2, 3, and 4 gave encouraging results for Gram-positive bacteria and acted as bactericide against S. epidermidis as well as E. faecalis (ATCC 29212) and its clinical isolate. R. officinalis led to promising results in the case of Gram-positive bacteria, resulting in a considerable interest in the development of reliable alternatives for the treatment of urinary infections.

O uso da dapsona é frequentemente associado a efeitos adversos hematológicos, como a metemoglobinemia e anemia hemolítica, ambos relacionados com a N-hidroxilação mediada pelo sistema P450. O objetivo do estudo foi avaliar a influência da suplementação de L-arginina, um precursor da síntese de óxido nítrico, administrado em regime de dose única ou múltipla na metemoglobinemia induzida pela dapsona. Ratos machos Wistar foram tratados com L-arginina (po, gavagem) em dose única ou múltipla de 5, 15, 30, 60 e 180 mg/kg 2 horas antes da administração de dapsona (40 mg/kg, ip). O efeito do L-NAME, um inibidor de óxido nítrico sintase (NOS), foi avaliado através do tratamento com doses múltiplas de 30 mg/kg. Amostras de sangue foram coletadas duas horas após a administração de dapsona. A concentração de metemoglobina eritrocitária foi analisada por espectrofotometria. Os resultados mostraram que a suplementação em dose múltipla de 5 e 15 mg/kg de L-arginina reduziu os níveis de metemoglobina induzida pela dapsona. Este efeito é mediado pela formação de óxido nítrico, uma vez que a redução nos níveis de metemoglobina pela L-arginina é bloqueada pela administração simultânea de L-NAME, um inibidor da óxido nítrico sintase.

Dapsone use is frequently associated to hematological side effects such as methemoglobinemia and hemolytic anemia, which are related to N-hydroxylation mediated by the P450 enzyme system. The aim of the present study was to evaluate the influence of L-arginine supplementation, a precursor for the synthesis of nitric oxide, as single or multiple dose regimens on dapsone-induced methemoglobinemia. Male Wistar rats were treated with L-arginine at 5, 15, 30, 60 and 180 mg/kg doses (p.o., gavage) in single or multiple dose regimens 2 hours prior to dapsone administration (40 mg/kg, i.p.). The effect of the nitric oxide synthase inhibitor L-NAME was investigated by treatment with multiple doses of 30 mg/kg (p.o., gavage) 2 hours before dapsone administration. Blood samples were collected 2 hours after dapsone administration. Erythrocytic methemoglobin levels were assayed by spectrophotometry. The results showed that multiple dose supplementations with 5 and 15 mg/kg L-arginine reduced dapsone-induced methemoglobin levels. This effect is mediated by nitric oxide formation, since the reduction in methemoglobin levels by L-arginine is blocked by simultaneous administration with L-NAME, a nitric oxide synthase inhibitor.

Candida parapsilosis, currently divided into three distinct species, proliferates in glucose-rich solutions and has been associated with infections resulting from the use of medical devices made of plastic, an environment common in dialysis centres. The aims of this study were (i) to screen for Candida orthopsilosis and Candida metapsilosis (100 environmental isolates previously identified as C. parapsilosis), (ii) to test the ability of these isolates to form biofilm and (iii) to investigate the in vitro susceptibility of Candida spp biofilms to the antifungal agents, fluconazole (FLC) and amphotericin B (AMB). Isolates were obtained from a hydraulic circuit collected from a haemodialysis unit. Based on molecular criteria, 47 strains were re-identified as C. orthopsilosis and 53 as C. parapsilosis. Analyses using a formazan salt reduction assay and total viable count, together with microscopy studies, revealed that 72 strains were able to form biofilm that was structurally similar, but with minor differences in morphology. A microtitre-based colorimetric assay used to test the susceptibility of fungal biofilms to AMB and FLC demonstrated that the C. parapsilosis complex displayed an increased resistance to these antifungal agents. The results from these analyses may provide a basis for implementing quality controls and monitoring to ensure the microbiological purity of dialysis water, including the presence of yeast.

The quality of mineral water commercialized in Brazil regarding the microbial content was analyzed and the results were compared with the standards established by the current legislation. Results demonstrated there was no bacterial contamination, but several types of fungi were found. Therefore, bottled mineral water could be considered a possible route for the transmission of filamentous fungi and yeasts.
